Your Rights
Subject to applicable local law, you may contact AstraZeneca to request access to the personal data we hold about you, to correct any mistakes, or to request deletion or suspend the use of the same. If such a request places AstraZeneca or its affiliates in breach of its obligations under applicable laws, regulations or codes of practice, then AstraZeneca may not be able to comply with your request but you may still be able to request that we block the use of your personal information for further processing. You may also have a right to data portability to another Data Controller under certain circumstances.

Proof of identity
The form asks you to give information about yourself that will help AstraZeneca to confirm your identity. AstraZeneca has a duty to ensure that information is held in a secure manner and as such we must be satisfied that you are who you say you are before we respond to any request.You may be required to provide identification documentation to validate your identity and/or authority to act on another's behalf.After they have been checked, any original identification document(s) will be returned, photocopies of identification documents will be destroyed, and any electronic copies will be securely deleted.
Retention
Personal data related to a subject access request will be retained for 3 years after the last activity on the request. Should such information later become relevant in relation to ongoing or potential legal proceedings, information may be preserved, under the authority of the Company Legal Counsel, until the matter is resolved and the hold is officially released.
